Reporting an Incident

If you attend the relevant training and follow the health an safety procedures most your sessions will run without the incident. But if there is a medical incident we need to know. If you are not sure if you should report an incident, report it anyway. As committee members, it is important you never keep your concerns to yourself. If you are concerned about a situation or a student always speak to the DSU Opportunities or Advice team. 

Please complete the form below as soon as possible after the student has had an accident or a near miss.
